Accessing the Care You Need
Insurance
Out-of-Network
PHL Therapy does not participate with insurance companies but will provide you with a statement of services containing the necessary billing codes that you can submit to your insurance company for out-of-network reimbursement.
Additionally, they accept HSA and FSA funds as a form of payment.
Please check with your insurance company about the reimbursement benefit. If you wish to submit services through insurance, it is your responsibility to send the required information to your insurance company directly.
Sliding scale fees are available on a limited basis; please inquire for more information.
Payment
Due at time of service
It is the responsibility of the client to provide payment at the time of service. You can request to receive a receipt for your personal and/or insurance needs. If needed, special payment arrangements may be discussed prior to the start of the session.
Cancellation & Rescheduling
Please contact your therapist as soon as possible if you need to cancel or reschedule a session.
Cancellations and reschedules made less than 24 hours before therapy sessions will result in a full session charge.
